Rate Breakdown: - $50/hour: Basic follow-ups, scheduling, paperwork - $100/hour: Legal/medical advocacy, elder/disability care navigation
Includes:- Behavioral/mental health service linkage - Crisis planning & stabilization coordination - Community referrals (food, housing, therapy) - Ongoing social support, including caregiver resources
Rate Breakdown: - $60/hour: General coordination and referral - $120/hour: High-intensity social work or crisis management
Includes:- Appointment accompaniment - Daily check-ins and non-clinical wellness visits - Light errands, meal prep, and medication reminders
Rate Breakdown: - $30/hour: Routine assistance or scheduled visits - $40/hour: Weekends/evenings or rural area travel
Includes:- 60–90 minute virtual or in-home intake visit - Medical/social history review - Needs assessment and written plan of care
Rate Breakdown: - $150: Phone or video consultation only - $300: Comprehensive in-person intake and documentation
Includes: - Ongoing case management with assigned care manager - Access to Trident’s client support line - Regular updates to family or POA - Monthly summary reports and care coordination
Rate Breakdown: - $500/month: Basic care oversight (8 hours/month) - $1,200/month: High-intensity case management (16+ hours/month)
